Hi all,
As a soon to be qualified practitioner, I've read a few of the threads on the whys and wherefores of taking detailed personal historys.
I wonder if anyone knows whereabouts these questions can be found online as I'd very much like to find a proforma for this to support what we have studied.

What country are you in? The National Guild of Hypnotists has two basic forms you can download and change into NLP. One is a name, address, phone number type of form and the other is "The Client's Bill of Rights" which could be worked over for NLP if there are regulations on NLP where you live. Personally, I don't go beyond basic contact information.

I would suggest going to your regular doctor and asking for a copy of their Patient Intake Form.

I'd suggest that any intake forms used by those that are non-medical
professionals be significantly different than those used by doctors.
SG you're right, of course.
What I should have said is that having such a form would be a good place to start to design your own.
I suspect it would be easier to tell us what your school suggests, and we can then make additional suggestions as needed.

The question makes no sense.
If you are studying NLP you should know what questions you have to ask. Ask questions that will be useful. If you do not know what questions you need to ask you need to do more studying.

Hia - I remember when I had done my NLP Practitioner certification and some of us were booked to do the Master Prac. we knew of the 'elusive' detailed personal history and saw it as the 'gateway' to Breakthrough Session work............
........ as it turned out, we were not really ready to work with the level of depth of issues that such was to be used for and so waiting until the relevant point of our Master Practitioner training was arguably better for us.
Hence - the question(s) above about what your trainers suggest is so valid.
Have they suggested any such thing to you yet? What are you wanting to use it for at this stage of your training?
