View Full Version : Client Forms for Hypnotherapy, NLP, & Time Line in Florida

Help,

Are there any existing forms for client application and information for initial visits acceptible for the State of Florida,. I am opening a new practice and seek existing tried and true forms, instead of reinventing the wheel.

The forms should contain:
client consent to treatment
client history
privacy policies statements
liability info
payment policies
etc.

If nobody has them, since you have a computer, I would respectfully suggest that you consider designing them yourself. You could print them up and offer them for sale on your web own website.

I have received information from the ABH and NGH on such forms for California (which has a specific law as to what needs to be included) and I know of at least one person who sells packages of such forms for Californians.

hello,

Unless you are licensed you cannot do therapy in Florida. You xcan do hypnosis, but not hypnotherapy
Laws are very strict in Florida. You should contact the NGH to check on Florida law They may be able to help you with the appropriate forms too

Both the NGH and the ABH provide the information you need to use for free. I would suggest contacting those organizations.

Since the site I mentioned sells the forms, that would be advertising, and we don't do that on this forum. I have sent you a private message with the information.

The site mentioned, however, does say the following:

NEW LAW FOR ALL California MIND, BODY, SPIRIT WORKERS
Alternative Healthcare Disclosure Forms are Now Required for Coaches, Counselors, Hypnotists, NLPers, Spiritual Counselors, Pastoral Counselors, Therapists, Nutritionists, Naturopaths, Bodyworkers, Skincare Specialists & Movement Therapists!

As of January 1, 2003, the California Business & Professions Code requires that client’s of non-licensed practitioners sign a written Complimentary & Alternative Health Care form at their first visit. One copy is to be given to them & the other is to remain in your records for three years. This form is said to protect you from “being cited, penalized or fined for violating the Medical Practice Act of California.” Other states are considering similar legislation.

P.S. Note that the new code also states: “A person who advertises such lawful (alternative) services must say that he or she is ‘not licensed by the state as a healing arts practitioner’” (I would add: “but is certified in their field of expertise”)

[To which I, Don, would add, if you are certified, something I would highly recommend. Certification costs little, shows that you are trained and educated, continue your training, and supports the hypnosis community. I am certified by both the ABH and NGH.]

Remember, the Californa law goes [went] into effect January 1st, 2003.

What specifically needs to be included on this form in California in order to legally comply?

As I understand it (and anybody who knows more, please correct me if I'm wrong), it must include:
1) That you are NOT licensed by the state
2) Your certifications, training, and experience
3) What you are going to be doing and the theory behind it.

Both you and the client must sign copies of it. They get a copy to take with them and you must keep your copy of their form for at least 3 years.
